By signing the Delivery Room Data Form, the individual collecting the cord blood attests to the fact that they understand the collection requirements and collection guidelines were followed during the collection.

CBU collection should not be initiated if the following conditions exist: no maternal consent; concern of misidentification; and/or collection would interfere with the care of the mother or baby.
